What is your status in the SOAR program?

SOAR's ability to provide transportation is extremely limited. Will youth need transportation in order to participate?

No - Youth will NOT NEED transportation from a SOAR staff. Youth has their own transportation from a family member/friend.

Yes - Youth WILL NEED transportation from a SOAR staff. I understand that SOAR’s ability to provide transportation is extremely limited and cannot be guaranteed.

Please write the name and phone number of family/friend who is authorized to pick up the youth if needed. SOAR staff will be unable to support transportation for youth wanting/needing to leave early.
